Medication scan: closing the reconciliation gap at every visit

MMurphi TeamMurphi.ai

Medication reconciliation is one of the most important and least loved tasks in clinical care. Patients forget names and doses, bring a bag of bottles or a handwritten list, and the chart is often out of date. Errors here lead directly to adverse events.

Point the camera

With Murphi's medication scan, a clinician, nurse or even the patient photographs a bottle, label or list with a phone. Murphi reads the medication name, strength and instructions and matches them against the current medication list in the EHR.

What gets flagged

Three situations matter most: dose changes, new medications not in the chart, and chart medications the patient is no longer taking. Murphi flags each clearly — "Lisinopril 10 mg, chart says 5 mg" — so the clinician can confirm the correct dose in seconds.

Interaction checks before sign-off

Newly discovered over-the-counter drugs are a common blind spot. An NSAID taken alongside an ACE inhibitor, for example, is worth reviewing. Murphi surfaces these combinations before the updated list is signed, when there's still time to act.

Where it fits

Medication scan is valuable at intake, during annual reviews, after hospital discharge, and in correctional health, where patients often arrive with medications from outside providers. Behavioral health teams use it to confirm psychiatric medications and doses across prescribers.

Writing it back

Once confirmed, the reconciled list is written back to the EHR and referenced in the visit note. No retyping, no separate form.

Reconciliation will always require clinical judgment. Medication scan simply gives that judgment accurate information to work with.
